Purpose of Position :

To provide comprehensive gender-responsive health care and nursing services to male and female youth of the Roy McMurtry Youth Centre and contribute to a case management multidisciplinary team approach in the provision of services to youth.

Duties / Responsibilities :

Under the general and professional supervision of the Manager of Health Care Services the position:

1. Performs a variety of professional nursing duties by: Interviewing and assessing all new youth admissions to ensure that all medical and nursing needs are identified, adequately addressed and that all youths receive individualized health care services Assisting physicians in medical examination of youth and assisting other professional health care providers, i.e. optometrist, dentist, X-ray, etc., as required Preparing and maintaining an effective medication dispensing and delivery system by ensuring policies and procedures of medication management are followed and any issues/ concerns are brought to the attention of the Manager of Health Care Services; Administering treatments and tests as ordered by the physician; monitoring that medication is administered consistent with prescribed directions; Interviewing youth who are confined due to sickness; monitoring progress and medical treatment as required or instructed by other professional medical staff; observing youth for any change in condition and updating the physician and/or facility staff Ensuring timely response, follow up, implementation of care and ongoing assessment of nursing and medical concerns expressed by the youth and facility staff; Ensuring timely response to youth - emergencies by providing first aid, completing necessary referrals to hospitals or facilitating emergency calls for physicians; Ensuring timely response to staff / employees emergencies by providing first aid and completing necessary referrals to hospitals or directing to health care services Preparing and maintaining confidential health care records including timely and accurate documentation, accurate filing of medical histories, reports and consultations; Compiling and filing accident and injury/occurrence reports where appropriate; Maintaining and documenting timely and accurate information in the logbooks ensuring confidentially Communicating pertinent and appropriate medical information to clinicians/physicians/ facility staff to ensure a continuum of care and effective health care approaches Collaborating with the Medical Clerk to ensure timely follow up and scheduling of clinics and procedures (Physician, Dental, Optometrist, and X-ray etc). Follow through with timely communication to the Youth Services Managers/ Shift Managers/Operation Managers regarding appointments Consulting and providing staff with specific instructions regarding the provision of special meals, medications, treatments, etc. for youth

2. Provides health education for youth by: Developing, leading and instructing gender-responsive health and wellness educational programs for youth; Counseling and providing appropriate information to individual youth regarding health issues or concerns

3. Participates within the case management process by: Providing health care assessment and intervention recommendations; working collaboratively with multidisciplinary team to meet identified needs of male and female youth; and assisting with relevant health care reintegration planning.

Staffing and Licensing :

Current unencumbered general registration as a Registered Nurse with the College of Nurses of Ontario. Current First Aid and CPR certification.

Knowledge :

Job requires knowledge of general nursing theory, practices and procedures and College of Nurses of Ontario standard of practice to provide a variety of nursing services. Job requires knowledge of facility and health care policies and procedures to ensure appropriate treatment implementation including safety of youth, and security of health care area; to report observations to the manager that may affect programs and youth activities. Job requires knowledge of infection control practices and skills/ability to act effectively on assessment outcomes. Job requires knowledge of relevant sections of related legislation (e.g. Regulated Health Care Professional Act, Health Care Consent Act, YCJA, CFSA) to ensure compliance with health care standards and to ensure that legislated youth entitlements are adhered to (e.g., consent to treatment, confidentiality of records). Ability to acquire knowledge of computerized Offender Tracking Information System (Youth OTIS) to input/retrieve young person information.

Skills :

Job requires effective assessment, judgment, reasoning, analytical and problem-solving skills to elicit and interpret information from youth and other sources, identify health care problems, evaluate information and draw conclusions to assist in the development of health care treatment plans, evaluate youths' response to planned health care intervention and to decide, in conjunction with youth, health care and facility team professionals whether to continue or modify interventions and youth treatment goals, to assess emergency situations and to decide on implementation of appropriate treatment measures. Job requires oral and written communication skills including terminology pertinent to the health care profession to present information (e.g. assessment of needs and treatment updates) and to collaborate with physicians or facility staff regarding youth health care, treatment and discharge plans; to advocate on behalf of youth with treatment professionals and agencies; to document/chart youth treatment and progress. Job requires interpersonal skills to advise/counsel youth about health care treatment, possible side effects of medication/health care treatment and expected outcomes; to promote education and achievement of treatment goals; to work effectively with health care team and multidisciplinary facility team, to interpret or clarify information about health care treatment to youth, facility staff, respond to complaints or concerns and to encourage youth compliance with health care treatment plans. Job requires ability to operate Microsoft office software, internet and email to prepare reports, enter and retrieve data and facilitate communication.

Freedom of Action :

Job requires working independently within parameters set by physician treatment directions, Ministry policies and procedures, legislation and College of Nurse of Ontario standards of practice to provide nursing services (e.g., implementing treatment process, first aid/emergency techniques, administering medication), ensuring safety of youth and security of health care area. Job evaluates health care situations in the absence of the physician, deciding upon course of action and referring doubtful or serious cases to physician, Manager of Health Care Services or hospital. Job requires communicating issues such as non-compliance with nursing standards, missing medication, errors and security concerns to supervising nurse, physician or manager.
