Purpose of Position :
To provide professional gender-responsive social work services and coordinate and facilitate the case management process for young persons at the
Cecil Facer Youth Centre
Duties / Responsibilities :
The position provides social work and case management services under the general supervision of the Supervisor, Social Work Services by:
Providing professional social work services:
' Conducting intake and risk assessments
' Developing, implementing, facilitating and evaluating gender responsive best practice/evidence based treatment programs
' Providing gender responsive individual, group, and family counseling
' Providing crisis intervention services
' Consulting and collaborating with community agencies
' Evaluating data and test results in order to make appropriate treatment recommendations
' Providing clinical transitional services to assist youth in community reintegration
Assuming Case Management Coordinator responsibilities in coordinating and contributing to effective case management by:
' Consulting with, coaching and maintaining regular contact with Youth Service Officers regarding development, implementation and evaluation of case
management plans, process and program implementation
' Maintaining a working knowledge of all young person's progress within assigned unit
' Meeting regularly with Youth Service Managers
' Consulting and collaborating with the multidisciplinary Case Management team
' Coordinating, facilitating and participating in the case management process, including case management meetings
' Ensuring implementation of the recommended case management plan
' Reviewing case management plans and other case management reports to ensure completeness, accuracy and compliance to time frames
' Assisting with the identification of youth needs, assessing skill development and progress
' Providing supports and programs that contribute to goal achievement
' Providing input/recommendations to reintegration plans and reintegration leaves by identifying progress, goal attainment and evolving needs
' Preparing reports as required by the case management process
' Liaising with family and community resources
Performing other related duties, such as:
' Participating in various committees and meetings as assigned
' Developing and facilitating staff training as required
' Supervising social work students
' Maintaining statistical reports as required
' Ensuring security and safety procedures are adhered to during social work programs and appointments
Staffing and Licensing :
Master of Social Work Degree or Bachelor of Social Work degree with significant post graduate supervised social work experience; Registration in good
standing with Ontario College of Social Workers and Social Service Workers
Knowledge :
Job requires in depth knowledge of theories, principles and practices of social work assessment and intervention skills acquired through a Bachelor of
Social Work (B.S.W.) or Master of Social Work (M.S.W.).
In depth knowledge of adolescent development, processes, theories and effective therapeutic interventions, including knowledge of gender specific
programming and understanding of relevant issues related to at risk youth to develop and implement treatment plans, monitor progress, and make
appropriate treatment recommendations; and to develop, implement and facilitate therapeutic treatment programs for youth
Knowledge of risk, need, and responsivity issues
Knowledge and ability to develop, implement and evaluate gender responsive best practice/evidence based approaches and interventions
Knowledge and expertise in case management to effectively coordinate and contribute to the case management process,
Knowledge of assessment processes and tools to conduct assessments, interpret assessment tools/tests.
Knowledge of relevant legislation; e.g., Youth Criminal Justice Act, Child Youth and Family Services Act, facility/ ministry policies and procedures and
rules of confidentiality.
Knowledge of community and program support services to assist youth in formulating reintegration plans.
Knowledge of computers and related software; e.g., word-processing software, database applications and internet to complete reports, risk
assessments, track statistical data, conduct research and for communication.
Skills :
Job requires analytical and problem solving skills in order to: to gather and compile data from multiple sources; e.g., youth, file history, ministry staff,
family, other external treatment professionals; identify youth needs; develop and/or modify rehabilitative, treatment and case management plans in
consultation with youth and case management team; to assess youth in crisis and apply appropriate therapeutic interventions; evaluate program and
treatment/services
Job requires excellent oral and written communication skills to prepare and carry out assessments, to complete case management plans and progress
reports, to offer individual, group and family counseling sessions with youth and other concerned parties (e.g. family), to discuss issues or problematic
cases with the Manager and other professionals, and to liaise with facility staff and/or community support services.
Job requires interpersonal skills to advocate on behalf of youth in the facility or community, when motivating and counseling youth, and other concerned
parties (e.g. family); when coaching/facilitating case management team members
Job requires effective planning and organizational skills to manage workload meet requirements of case management process and prepare
reintegration plans.
Job requires presentation, facilitation and group leadership skills to provide group counseling, treatment programs, staff training and coaching.
Job requires ability to work effectively within a multidisciplinary team for the purposes of case management.
Freedom of Action :
Job requires working in accordance with established program policies and procedures, to write reports, to provide care to youth, and provide information
about social work services to facility staff.
Job requires working within guidelines of relevant legislation and recognized and approved professional ethics and practices, and maintaining
confidentiality.
Job requires discussing development and modification of rehabilitative treatment and case management plans with youth and case management team,
and as necessary with the Manager of Social Work Services.
Job requires providing updates to the Supervisor of Social Work Services, who is consulted on unusual cases and situations not covered by established
methods and procedure.
Job requires exercising independence and using professional discretion when working with youth.
