Purpose of Position :

To provide case management services for clients receiving services at CPRI.

Duties / Responsibilities :

Working as a member of the Clinical Services Program as part of a multidisciplinary team the incumbent is responsible for case management of cases which includes:

- Working in conjunction with caregivers, client's community professionals and the CPRI clinical team to take responsibility for the co-ordination of the client service process.
- Attending, convening, chairing and organizing conferences/meetings related to client services while ensuring that client's caregivers and communities are involved in the process.
- Coordinating and completing assessment tools such as interRAI
- Completing required documentation related to client service (i.e. plans of care, reports. etc.).
- Ensuring guidelines and timelines are followed.
- Ensuring recommendations from treatment and assessments are documented and communicated to client's caregivers and involved community agencies.
- Collaborating with community case managers and agencies to assist in accessing resources to meet client's/caregiver's needs.
- Completing relevant research and evaluation data as required.
- Providing a consistent, professional point of contact emanating from families, community professionals, agencies and CPRI staff.

Knowledge :

-Successful completion of a Social Services Worker, Child and Youth Worker, Developmental Service Worker or equivalent
-Experience in a social services setting or equivalent combination of experience and education in the children services system and the treatment of children with mental health and/or developmental needs.
-Knowledge of relevant psychological theories, child behaviour assessments, developmental and or mental health disorders, child development and social work systems theory.
-Knowledge of relevant child services legislation, including the Child and Family Services Act, Freedom of Information and Protection of Privacy Act and related government and facility policies.
-Knowledge of assessment methods and function and relevance of clinical assessments.

Skills :

-Computer skills (i.e. Microsoft Word, Excel and Outlook)
-Ability to co-ordinate treatment information and collect data to produce relevant reports and conduct appropriate statistical analyses
-Communication (verbal and written), organizational and presentation skills to prepare reports and training materials in an accurate and concise manner and to relate to a variety of people and adapt to a variety of roles
-Reasoning and analytical skills to gather relevant information to discuss treatment needs with referral sources and families
-Excellent organization skills to co-ordinate and manage information and data from a wide variety of sources and to plan and prioritize own workload within established timelines
-Excellent interpersonal skills to work collaboratively with a variety of professionals, staff, members of the public and community representatives
-Ability to travel within the service area
-Ability to interview clients in order to complete assessment tools

Freedom of Action :

This position works under the general direction of the Program Support Manager. The job exercises latitude in decision-making by planning, conducting and working in conjunction with caregivers and community professionals, i.e. hospitals, children's agencies. This job requires working independently and as part of a multidisciplinary team: prioritizing and coordinating own workload to meet the needs of clients and stakeholders. Work is performed within residential, ministry, legislative and relevant College Standards.
