About the job
Working in a multi-disciplinary team, you will:
• Provide a full range of professional, culturally, and gender-responsive social work services
• Coordinate and facilitate the case management process
• Provide clinical assessment, crisis intervention and specialized treatment services for complex cases
• Provide input into individual goal development
• Develop and deliver evidence-based programs
• Conduct individual, family and group counseling and treatment sessions
• Liaise with family and community organizations
What you bring to the team
Mandatory requirements
• Master of Social Work or Bachelor of Social Work degree with significant post graduate supervised social work experience
• Currently registered in good standing as a Social Worker with the Ontario College of Social Workers and Social Service Workers
Specialized Knowledge and Experience
• You have in-depth knowledge of theories, principles, and practices of social work assessment and intervention
• You can develop and implement rehabilitative treatment plans and programs to clients with complex needs
• You have expertise in case management and clinical planning
• You have demonstrated knowledge of adolescent development, processes, theories and effective therapeutic interventions including understanding of gender specific programming and relevant issues related to at risk youth
• You have knowledge of relevant community resources and agencies
Proven Communication, Presentation, Facilitation, Interpersonal and Group Leadership Skills
• You can provide training, assessment services, individual, family and group counseling programs
• You are able to adjust your communication in order to connect and build rapport with youth who have diverse/varied cultural, ethnic, and educational backgrounds
• You can prepare written assessments and program evaluations
• You have experience working effectively within a multidisciplinary team including external agencies
Analytical and Problem-Solving Skills
• You can diagnose youth risk, needs, and responsivity issues
• You can develop treatment plans and evaluate programs and services
• You have knowledge of and ability to apply relevant legislation
• You have organizational skills and planning skills to manage workload
Computer Skills
• You are proficient with computers including word processing, databases, and internet applications to complete reports, risk assessments, and track statistical data
