A rewarding career awaits you at the Ministry of Education, Provincial and Demonstration Schools Branch. Currently, we are looking for (1) Seasonal, Case Management Consultant to provide a range of case management consulting supports to students and families enrolled in Provincial and Demonstration Schools. This position will provide admission assessments, develop transitional care plans, and liaise with internal program staff, external District school boards and community agencies. This position will provide a blend of virtual, and in-person visits to all school sites across PDSB.

Note: The successful candidate will report to one of the following work locations:
- Sir James Whitney School for the Deaf (Belleville Campus) – 350 Dundas St W., Belleville
- W. Ross Macdonald School for the Blind (Brantford Campus) – 350 Brant Ave, Brantford
- Robarts School for the Deaf (London Campus) – 1515 Cheapside Drive, London
- Ernest C. Drury School for the Deaf (Milton Campus) – 255 Ontario St S., Milton

What can I expect to do in this role?

In this role, you will:
  • Assess student needs, provide feedback and service plan recommendations and contract for service delivery for students participating in the program.
  • Provide transition case management services to students and parents, to facilitate the integration of the students' needs into the program, school and/or school board.
  • Provide guidance to families and students to facilitate community resource engagement.
  • Coordinate community services and liaise with other professionals involved.
  • Participate in ongoing evaluation of the services to clients and students.
  • Maintain and coordinate a resource lending library of pamphlets, books and visual resources.
  • Use program procedures to promote professional accountability and engage team members in treatment and program development for students.


How do I qualify?

Mandatory

  • Valid Ontario Driver's License to travel to different school sites throughout the Province of Ontario.
  • Registered Social Services Worker in good standing with the Ontario College of Social Worker and Social Service Workers or Child and Youth Worker in good standing with the Ontario Association of Child and Youth Care.


Knowledge and Abilities:

  • In depth knowledge working with youth in a recognized educational or lodging setting.
  • Completed training to understand and support youth and their families' social and emotional needs.
  • Demonstrated ability to manage complex cases with multiple ministries and government agencies.
  • Knowledge of relevant legislation, ministry and Provincial and Demonstration schools branch policies, procedures and guidelines.
  • Ability to provide guidance to families and students by applying knowledge of child development and rules around confidentiality.
  • Knowledge of educational programs, community resources and family support systems to meet the needs of the students.
  • Demonstrated counselling and case management skills.


Additional Skills:

  • Proficiency in American Sign Language (ASL) to support students who are Deaf/Hard of Hearing and/or willingness to obtain.
  • Ability to communicate using hand over hand communication and/or Braille to support students who are Blind/Low Vision, Deaf-Blind or willingness to obtain.
  • Advanced interpersonal, verbal and written communication skills in English, to communicate effectively with parents, colleagues, and professionals and prepare well-developed documentation.
  • Well-developed research and recording skills for documentation.
  • Excellent planning and organizational skills.
